Puppy Reservation & Policy Form

 

 

Puppies are reserved with a $100 Non-refundable deposit ( if paying through Paypal please add a $5 processing fee). When the owner makes the deposit, the date will be noted and he/she will be notified of picking order. Upon sending in the deposit, all owners should submit a preferred color and sex of the puppy. If by chance the puppy is injured or dies the deposit will be refunded, otherwise Northwoods Labs has no obligation to return a deposit.  Puppies are sold with a wellness exam given at 6 weeks and determined healthy at time of sale.  New owner assumes all responsibility of puppy after transfer of ownership. Balance of payment due at transfer.
